Transition Roadmap

Ciag Flow

Outcomes

Transition planning begins from the moment our learners start their programme of study. We are committed to supporting our learners into aspirational and sustainable outcomes – this outcome will look different for each individual, but can be largely grouped into the following categories:

Employment

E.g., paid, voluntary or supported voluntary.

Social

E.g., day services, increased independence at home or in the local community, personal assistant support, etc.

Further Education

E.g., further education at another college, apprenticeship, etc.

Local Options

If you are looking for more information about services linked to transition available in your local area, a good place to start is with the Local Offer, or by talking to our friendly transitions team on 01246 433 788. Additionally, you can reach out via email to elizabethwalker@landmarks.ac.uk for further enquiries.
