Leaving Landmarks is an exciting next step, but we know it can also feel like a big change. Our Transition Team is here to help and support you as you plan for life after college.

We’ll work with you to explore your options, talk through ideas and help you decide what you’d like your next step to look like.

This could include:

  • Finding employment or a supported internship

  • Exploring further education or training

  • Exploring education and learning opportunities

  • Looking at volunteering opportunities

  • Finding the right day service or support

  • Exploring independent living options

  • Developing skills for greater independence

Our Transition Team can help you understand your options, make connections with the right organisations and support you towards your next destination.
